At Well Spine Chiropractic, we welcome patients referred under Medicare-funded Enhanced Primary Care (EPC) or Chronic Disease Management (CDM) plans.
These plans are designed to support people living with chronic or complex health conditions who may benefit from a coordinated approach to healthcare. If your GP has prepared a valid EPC or CDM referral, you may be eligible to receive Medicare rebates for a limited number of chiropractic consultations each calendar year.
Our chiropractors work closely with your GP and other healthcare providers to help you manage musculoskeletal conditions, improve mobility, reduce pain, and support your overall health goals.

What to Bring to Your Appointment
• A valid EPC/CDM referral from your GP
• Your Medicare card
• Any relevant scans, reports, or medical information
Our team can assist with Medicare claiming and provide progress reports back to your GP when required.
